ONONDAGA COUNTY, N.Y. -- Authorities have identified the driver of a dump truck who died in an accident in the town of Manlius. It happened at the Kinsella Quarry off Route 5 just after 11 a.m. Thursday.

Police say two people were inside the privately-owned dump truck, which was loaded with gravel, when it went off the road and hit an embankment. Investigators believe that a mechanical problem may have forced the men to jump out of the truck while it was moving. A passenger was able to jump out with only minor injuries.

The driver, Terry Johnson, 49, was killed when he either jumped or fell from the truck.

State Police and OSHA are continuing to investigate the accident.
